Client Intake Form/Cancellation Policy
All new clients are asked to print this form, complete it and bring it with you to your first session. Any client who has not been seen by Jennifer in the last 12 months will be asked to complete another form. If you have a particular condition or health concern and want to make sure massage is indicated, please contact Jennifer prior to your appointment.
